Have you asked him what he thinks about the situation? Why does he think the panhandler is homeless? Why does he think the panhandler doesn't have any food? Is he worried for them? Does he want to help them? Sometimes the best way to talk to a kid is to help them talk to you.

The -1 is for "I don't a know single baby that has not had a diaper rash." I don't either, but I haven't ever had a doctor tell me that a rash was a bacterial infection, either. I don't think that dismissing what could be a serious medical concern is particularly helpful. As a side note, my underwear preferences would be heavily biased against cotton if I got blisters on my penis every time I wore cotton underwear.
