Toilet training is a good thing to try, to get some regularity in the process of eating and digestion. Toilet training makes most sense (according to "our" specialized doctor - we have a similar problem, the boy is 6.5 years old now) directly or short after meals: after each meal (breakfast, lunch, dinner) the child should sit on the toilet for 5..10 minutes. Input stimulates output, so to say.

Was there a longer time in his life when he successfully went to the toilet or is this a continuous state for you and him? I understand your pain, we have a similar problem, we're just 2 years "behind", see here: parenting.stackexchange.com/questions/2209/…

@ShadowWizard: thanks for your comment! I'm not sure if a diaper would really solve the problem, because he still has to clean himself (the main problem) and it will also smell. The only advantage of a diaper would be a "throw-away-underwear", but on the other hand the risk is immensely great, that if the other children find out about it, he would be even more isolated and made fun of.

@JeremyMiller: the diagnosis of a doctor specialized in this kind of problems is "encopresis with obstipation". And you're absolutely right: this question is unfortunately about how to live with the results, as we still have not yet managed to find and remove the cause (and I read about other people who had to live with that up into the teenage years...)

firstly, thanks a lot for asking this difficult question and giving so many details. I wish you and your family all the best. This is not an answer to your question, but did you try any legal ways for forbidding your uncle/mother to be in contact with your children? I'm not sure if this would be possible (in your country - I neither would know about mine) and if that might help.

it really really depends on the child, if that is possible. With our children (during the phase when they absolutely wanted to also touch the computer) they were permanently with their hands on the keyboard or mouse - no way I would have been able to concentrate on my work (or even private things like reading/writing an e-mail) like that without getting frustrated or angry. So I tried to keep those things separate. However, I've heard from other parents who wrote their dissertation with the baby on the lap. That would not have been possible for us.
